The wellknown fiasco of the baggage handling system at the recently opened Denver airport, the failure of the Ariane5 space shuttle, and the failure of the computerised London Ambulance Service has raised many questions about the design and running of mission critical computer systems. This report traces the history of some major software projects and identifies the flaws that led to their failure. Could things have been done differently...?
